Queries, compliments and complaints

The Patient Advice and Liaison Service (PALS) is a service that deals with patients', relatives' or carers' queries and concerns regarding their health care, and can provide information on NHS services generally.

Media enquiries

All media enquiries should be directed to the Communications Team, by email media@covwarkpt.nhs.uk.

We are open from 9am to 5pm, Monday to Friday.

For urgent media enquiries outside office hours, including weekends and bank holidays, contact the St Michael's switchboard: 0300 303 3664.

Access to your Health Records

If you wish to request access to personal information held about you, please contact us via email at mlcsusars@nhs.net and include "Subject Access Request" in the subject line.

All requests will be recorded, and you may need to provide information to verify your identity and enable us to locate the information. Please include the following details in your request:

  • Full name, address, date of birth, NHS number (requests for health records only)
  • What information you are requesting

Examples of acceptable identity evidence are listed below. We require, where applicable, two items from List A and one from List B.

List A (ID documents)

  • Birth certificate
  • Passport
  • Driving license
  • Staff ID badge (for NHS England and NHS Improvement employees only)

List B (proof of address)

  • Bank statement
  • Utility bill
  • Tax certificate
