Routine Outcome Measures
Routine Outcome Measures are questions that we will ask you before your first appointment with us, and throughout your time with us. Answering them helps us to see how you are doing. Some questions help to involve you in decisions about the care and the support you receive. Other questions give us an understanding of what you are feeling and the difficulties you are having.
Do I have to take part?
We will only collect this information if you say it is ok. If you do not want to answer the questions that's fine and we will still do our best to help just the same. The more information you give us about what you are feeling the easier it will be for us to understand and help you.
What will I need to do?
There will be different questions we may ask you. If they are written down we can help with the reading and also explain what the questions mean.
Parents/Carers will also be asked to fill in a questionnaire.
We will always be there to offer any help if you need it.
What happens to the information?
We keep all the information safe and private in your patient notes.
We might share the information with our team so they can help us think of the best thing to do to help you but only if you say it is ok.
We will ask you about this at your appointment.
