Special school nursing

  • The special school nursing team support the complex physical health needs of children and young people within special provision schools, which enables them to safely access their education. We do this by assessing, care planning and delivering daily essential clinical care to maintain optimal health. We also respond to any health deterioration by escalating care needs. We work collaboratively with the wider CYP community nursing team as well as other community professional and education colleagues. We are a team of both children’s carer and nurses. We advocate for the CYP and their families.

  • Referral Criteria: Any CYP diagnosed with complex physical health needs who attends one of the 5 supported schools.
