Coventry and Warwickshire Partnership NHS Trust to launch a new service to transform care for older adults in Coventry and Warwickshire
A new service for older adults with mental illness will enhance care across Coventry and Warwickshire, enabling high-quality, personalised treatment in the comfort of their own homes.
The new Hospital at Home service, due to become operational in August 2026, represents a major step forward in delivering modern, patient-centred care. Aligned with the NHS 10 year plan priority to move care from hospital to the community, the service is designed to provide clinical support at home as an alternative to hospital admission and aims to improve outcomes, promote independence, and support faster recovery for older adults.
This model is part of a wider commitment to strengthening care pathways and ensuring that people receive the right care, in the right place, at the right time and bridges the gap between Community Mental Health teams, Home Treatment teams and inpatient services, offering a new, responsive service, meeting the needs of older adults who are experiencing mental illness.
Delivering better care closer to home
Evidence from both local reviews and national programmes shows that older adults experience better outcomes when supported intensively at home or close to home. Patients benefit from remaining in familiar surroundings, maintaining connections with family and carers, and avoiding unnecessary hospital stays wherever clinically appropriate.
The new service has been shaped through extensive engagement with service users and carers. Their feedback has directly influenced the design, ensuring it is responsive, flexible, and tailored to the needs of the community. In addition, a review of clinical data highlighted a clear opportunity to prevent avoidable hospital admissions, an opportunity this new service will now address.
Care will be individually tailored, with patients receiving up to four visits per day depending on clinical need. Multidisciplinary teams will work closely together to provide safe, effective and compassionate care, while inpatient services across Coventry and Warwickshire will continue to be available where required.
Robust, safe, and flexible
The Trust plans to ensure the service delivers consistently high standards, by monitoring its effectiveness, reviewing patient outcomes, the number of older adults who require readmission, length of stay in hospital and patient and carer feedback. This approach will support continuous improvement, transparency, and rapid response if needed.
The introduction of Hospital at Home also brings increased flexibility to the support available to older people who are experiencing mental illness across Coventry and Warwickshire.
Supporting wider needs
While Hospital at Home is a clinically focused service, it also plays a vital role in supporting wider wellbeing. By enabling people to remain at home, the model helps maintain social connections and provides an opportunity to link individuals in with community and voluntary sector support where appropriate. Work will continue in partnership with local authorities and community organisations to address broader issues such as loneliness and social isolation.
The service will support a wide range of patient needs, including both functional mental health conditions, such as bipolar disorder, schizophrenia and severe depression, and organic conditions such as dementia. Evidence from other areas where similar services have already been implemented are already delivering improved patient experience and outcomes and reducing unnecessary hospital admissions.
