What we are delivering in year 2 (2026 to 2027)
During year 2, the Medicines Safety Improvement Programme (MedSIP) is moving from planning and system discovery to active implementation, testing and evaluation. Following extensive engagement across Coventry and Warwickshire, 4 priority change ideas have been selected for delivery across the Integrated Care System (ICS).
1. Quality medication reviews in primary care
Working with Primary Care Networks (PCNs) to improve medication reviews for people with a learning disability who are prescribed antipsychotic medicines. The programme will support person-centred reviews that consider the benefits, risks and ongoing need for treatment, with input from specialist services where required. 2. Improving and standardising Health Action Plans (HAPs)
Collaborating with health and care partners to improve the quality and consistency of Health Action Plans, ensuring they better support medication optimisation, holistic care planning and shared decision-making for people with a learning disability.
3. Strengthening integrated health and social care working
Improving multidisciplinary collaboration by bringing together learning disability services, primary care, pharmacy, social care and other partners to provide more coordinated and joined-up support. This includes greater integration between professional teams and opportunities for shared care planning.
4. Embedding pharmacy expertise within Care, Education and Treatment Reviews (CETRs)
Testing approaches to increase pharmacist involvement within CETRs, ensuring medicines optimisation and medication-related risks are considered as part of wider care planning and decision-making for individuals with complex needs.
Supporting improvement through data and quality improvement
Alongside these 4 priority change ideas, the programme will continue to use nationally developed data tools and measurement systems to identify patients who may benefit from review, understand prescribing patterns and monitor outcomes.
Quality improvement methodology will be used to test, refine and scale successful interventions across Coventry and Warwickshire, ensuring improvements are sustainable and deliver meaningful benefits for patients, carers and services.
