About us
Our aim is to support and empower families to meet their child's healthcare needs at home and to reduce hospital visits and admissions. We liaise with other health, education and social care professionals to co-ordinate and support the provision of care to children and young people with complex care needs. The team delivers end of life care to children which includes in the last few days of life 24 hour, visiting as needed.
Specialist practitioners include a Respiratory Nurse Specialist, Play Specialist, Epilepsy Specialist Nurse, and Palliative Care Nurse Specialist, and Clinical Leads.
