Service overview
Community mental health service for people experiencing psychosis.
A team of NHS and Social Care professionals working together with other teams, including hospital services and the Crisis teams, to provide care to people in the community who have been diagnosed with a form of psychosis, such as schizophrenia or bi-polar disorder. We provide treatment and signpost to other services depending on individual needs.
The service works using the Recovery Model to maximise a person's potential and personal recovery by addressing health and social needs such as medication; accessing support for work; education and leisure; and support in managing their day to day mental health condition.
Teams operate from:
- Avenue House,North Warwickshire,
- The Railings, Rugby;
- Warwick Resource Centre, Warwick;
- Ashton House, Leamington Spa;
- Stratford Health Centre, South Warwickshire;
- Swanswell Point, Coventry.
