The Community Diabetes Service is a dedicated team of health professionals who specialize in delivering support, advice and education on treatments and management to people living with diabetes.

The team consists of two GPs with special interests in specific areas of diabetes such as diabetes in ethnic populations; obesity, and; cardiovascular risk. This team liaises closely with secondary care to deliver care in the community that empowers people who are living with diabetes, their families and carers through education, information and support that are all designed around an individual’s needs.

Living with diabetes means that you will need to make adjustments to certain aspects of your lifestyle that will help you to maintain good health. We are here to provide you with the support, advice and information on how certain treatments and self-management that you need can help you to live the life that you want, without compromising your health and overall wellbeing.

Community Diabetes Service will help you to manage your diabetes, your way. We can support you in maintaining a healthy glycaemic level through treatment, advice and regular monitoring of your bloods. We also provide a Lipids Service, which can help you to stabilise and maintain your lipid levels through a combination of advice, education and regular appointments.
