  1. ADHD Annual Medication Review and Transitions Service

    A Multi-Disciplinary Team (MDT) Transitions Clinic which is consultant-led, to support patients from Rise with complex needs or still establishing effective medication treatment.   3. Allied Health Professionals Neonatal Service

    In-reach Allied Health Professionals (AHP) host provider service to Neonatal Units and Special Care Baby Units (SCBUs) across Coventry and Warwickshire, covering George Eliot Hospital, Warwick Hospital & University Hospital Coventry & Warwickshire. The AHP Team will be responsible for supporting the delivery of equitable, high quality, evidenced based practice across the Local Maternity and Neonatal Systems which will integrate across hospital sites consisting of Occupational Therapists, Physiotherapists, Speech & Language Therapists and Dietitians.

  7. Children and Young People’s Continuing Health Care (CYPCC)

    CYPCC refers to the provision of bespoke packages of care and support to Children and Young People (CYP) who have needs that cannot be met by existing universal or specialist services. An MDT-led assessment referring to CYPCC criteria is undertaken to ascertain eligibility. In Coventry, CYPCC health care packages are tailored to individual need and are provided by Registered Nurses and competent Carers from The Birches Respite Team. CYPCC packages aim to support parents or carers to meet their child’s health needs and/or ensure the CYP needs are met when they are accessing education.

  14. Crisis Resolution and Home Treatment Team

    We provide urgent care and treatment to individuals in Coventry who are experiencing a mental health crisis. Our service offers a compassionate and responsive alternative to hospital admission by delivering intensive support and treatment directly in the comfort and familiarity of patients’ own homes. This approach helps individuals remain in their communities, close to family and support networks, while receiving the care they need to recover safely.

  16. Development Centre - (Learning Disability)

    The Development Centre provides specialist day service opportunities to Adults with a learning disability and autism who have may also additional complex needs. It offers bespoke packages and therapeutic interventions utilising positive behaviour support and prides itself on its trauma informed and non-restrictive approach. The service promotes a collaborative approach and has a parents and carers group that provides advocacy for its service users.

  28. Learning Disability Team (Adult)

    We provide a person-centred community based service to adults with a learning disability in Coventry, Warwickshire and Solihull. Each of the five teams consist of Nursing, Psychiatry Psychology, Speech and Language Therapy, Occupational Therapy and Physiotherapy. Other services can be accessed depending on location, for example Art Therapy and Music Therapy.
