IPU (Integrated Practice Unit)
An IPU (Integrated Practice Unit) is a term used by the Trust to describe the way we group our services around you to meet your individual needs. Our services have been designed around Care Clusters to make sure that your care is delivered in a way that is right for you.
Referral to our services
You, your carer, GP or other agencies are able to make a referral to our services.
Each referral the Trust receives is managed through our central referral and triage process and, if you need an assessment by our team, an appointment will be allocated and booked for you by our Central Booking Service.
Assessment of your needs
You will receive an appointment, providing the date, time and place of your assessment, by post or by telephone. What you can expect from your assessment with us will also be provided and you are welcome to bring a friend or relative to support you to this appointment.
During your assessment, you will be asked questions about you, your mental and physical health, what difficulties you are experiencing now and in the past and discuss who the significant people are in your life.
This information will help our team of professionals, including psychiatrists, nurses, doctors, social workers, psychologists and occupational therapists identify the right treatment and support for you. This assessment will be undertaken once to improve access to the right care, first time, every time.
After your assessment is complete, you and your care co-ordinator will discuss the Care Cluster you have been allocated, develop a care plan and create a package of care, that is right for you, to ensure that you get the most effective treatment available.
