The Coventry and Warwickshire Integrated Care Record (CWICR)

Coventry and Warwickshire Partnership Trust is working in partnership with the other health and care organisations in Coventry and Warwickshire to develop an Integrated Care Record (ICR) for the population of Coventry and Warwickshire.

This will mean that any professional working for one of the partner organisations who is directly involved in your care will be able to view information from the other partner organisation to help with the way they are able to look after you.  This means that you should not have to remember your complete medical or medication history or keep answering the same questions each time you receive care from one of the organisations.

For the staff at Coventry and Warwickshire Partnership Trust this means that we will have access to information that should help us to provide you with even better care, even if that just means not asking you to repeat information time and time again.  It could also help us to make a more effective diagnosis and reduce the time from needing care to being offered the most appropriate care.

In August we will start rolling out access to the ICR for all relevant staff who are involved in your care and we will also start providing information to the ICR so that our partner organisations can start to use our records to help them care for you as well if needed.

You can find out more information about the ICR by visiting its website. You will find frequently asked questions and advice on what to do if you do not want your information to be viewed through the ICR.

Also please find below copies of a poster, leaflet and EasyRead document about the ICR you can download.
