New Service for Elderly Coventry Residents Holds Launch Event

Your health at home

More than 50 guests, including patient representatives, GPs, nurses, carers, hospitals and local authority members attended an event on Friday 24 June to officially launch a new community service for older people in Coventry.

The Your Health at Home service has been launched to help frail elderly people become more independent and safer within their own homes. The service provides care for people over 60 and focuses on improving and maintaining patient health and wellbeing.

The Coventry and Warwickshire Partnership NHS Trust held the launch event at Coventry Central Hall where attendees heard how health and social care professionals are working in partnership with the voluntary sector to overcome organisational barriers in order to meet patients’ care needs.

Your Health at Home has been rolled out across Coventry since January 2016. The care team is made up of health and social care professionals in the local area, including members of the NHS, City Council services and the voluntary sector.

Speakers at the event included Justine Richards, Director of Strategy and Business Support for the Trust, Dr Steve Allen, Chief Clinical Officer for the Coventry and Rugby Clinical Commissioning Group, Jasmine Osbourne, Team Leader for the Older People’s Team at Coventry City Council and Moira Pendlebury, Director of Services at Age UK.

Justine Richards from the Coventry & Warwickshire Partnership NHS Trust said:

“The Your Health at Home service is delivering an effective patient and professional experience. Patients are being seen by the appropriate people quickly and efficiently and are receiving a level of support which is more responsive to their needs.”

Jasmine Osbourne from Coventry City Council added:

“We are giving older people more choice and control over the way their support is being delivered. By working together, we can have a greater focus on prevention, wellbeing and self-care.”

The speeches were concluded by Garth Murphy, a patient representative who has been involved in the project since the beginning:

“The staff members working on this project are motivated and driven to improve the lives of their patients. They have shown a determination to work together to break down the barriers across the organisations involved in order to help those who need it.”

Operating in Coventry only, the service is co-ordinated by the Trust on behalf of Coventry and Rugby Clinical Commissioning Group (CCG) and working in partnership with the local authority and voluntary organisations, including Age UK.
