Transitions

Transition is the term used when a young person leaves Rise (the mental health service for children and young people) and their care is transferred to adult mental health services or third sector services.

This happens when a young person is 17, although the preparation for this transition will take place over a period of at least 6 months. This is so that the young person does not feel overwhelmed with the changes. It also gives them time to say goodbye to their worker in Rise and to meet the people they are going to be working with in the other services.

Their transition will be planned using a process called CPA. More information about CPA can be found on this website.

If it is felt that the young person's needs can be met outside of specialist services, they will be supported to access appropriate targeted services. This will also be part of the transitions process and arranged in a timely and organised way.

There is a specific person in Rise whose job it is to coordinate the transitions process. They will be able to answer any questions you may have about the process and they also offer consultation to staff in both Rise and adult services.
