Offer psychological assessment and intervention to people struggling to adjust to changes in their physical health. This may be through consultation and supervision with colleagues. Please note that this service does not accept self referrals (please see referral criteria below). Patients who require mental health support can access support via their GP practice, named healthcare professional or by contacting the mental health hub on 08081 866798.
