ADHD Annual Medication Review and Transitions Service

  • A Multi-Disciplinary Team (MDT) Transitions Clinic which is consultant-led, to support patients from Rise with complex needs or still establishing effective medication treatment. Also a nurse-led Annual Medication Review Clinic for patients with a non-complex ADHD need, who are in established medication treatment and under shared care with their GP.

Referral criteria: GP, our Trust’s Neurodevelopmental Team or NHS Children’s services.  All referred patients MUST have a (Rise or adult) NHS diagnosis from our Trust, and a Coventry or Warwickshire GP.


We provide a Multi-Disciplinary Team (MDT) Transitions Clinic which is consultant-led, to support patients from Rise (formerly CAMHS) with complex needs or still establishing effective medication treatment.

We provide a nurse-led Annual Medication Review Clinic for patients with a non-complex ADHD need, who are in established medication treatment and under shared care with their GP.

Please Note

Due to commissioning provision, we are unable to accept referrals for new patients over 25, patients with a private diagnosis, patients from out of area with an NHS ADHD diagnosis from a different NHS Trust or overseas diagnosis.

GPs would need to apply for out of area funding to their local neighbouring NHS Trusts to get a service for their patients.
